Q9 AND/OR S8 Stress Management System® Equipment Sales Agreement

Before we can ship your device you must read and sign this Sales Agreement.

(Hereinafter the “Agreement Date”). The Parties. This document was signed between

(Hereinafter the “Buyer”) and an Entity known as Seriously Smart Technologies (Hereinafter “SST” and “Seller”) at 4378 Wieman Rd, Beaverton MI 48612.

The Equipment to be sold is described as Q9 or S8 Stress Management System® for Stress Detection and Stress Reduction. (Hereinafter referred to as Q9/S8)

Sales Agreement Terms and Conditions.

The Q9/S8 is to be used for Stress Detection and Stress Reduction. No Medical Claims can be made. Only Synapse Report software is allowed to be used to give session reports to anyone who uses the Q9/S8. The Synapse Report has the best description of how the hardware and software function. The Q9/S8 is designed to be used by Members of properly formed Private Membership Associations, it is not designed to be used in the public domain. Should the Buyer or anyone in possession of the Q9/S8 use the device outside of the scope of this agreement, they do so at their own risk. SST and/or Smartronics Manufacturing cannot be held responsible for actions outside of the Private Membership Association. If Buyer sells or transfers ownership of the Q9/S8 without properly processing a transfer of ownership document with SST then the new buyer may not be able to use the Q9/S8 with full access to the Clasp64 software, and technical support. Proper Transfer of ownership is required.

Buyer and Seller agree that the S8 and Q9 is sold “as-is” with a 1-year Previously Owned Device Warranty or a 2-year New Device Warranty and a 6-month Harness Warranty made by SST as posted on the seriouslysmarttechnologies.com website on today’s date. The Warranty Policy as posted on the SST website on this date is the only warranty made by SST.
